What is Penile Curvature?
Penile curvature is the bending of the penis at a significant angle upwards, downwards, or to the sides during an erection. Not every curvature requires medical intervention; however, if the angle of curvature makes sexual intercourse difficult, causes pain, or creates significant aesthetic concern, surgical or medical treatment options should be evaluated.
Types and Causes of Penile Curvature
Curvatures are divided into two main groups based on their time of onset and mechanism:
Congenital Penile Curvature
- Cause: Anatomical differences such as one side of the spongy tissues (corpus cavernosum) developing more than the other or a short urinary canal (urethra).
- Course: It is usually painless, and the degree of curvature remains stable over time.
Acquired Curvature: Peyronie's Disease
Characterized by the formation of a hard plaque (scar tissue) in the sheath tissue of the penis (tunica albuginea), typically seen in men over the age of 40.
- Cause: Micro-traumas during sexual intercourse, genetic predisposition, or vascular issues can trigger the formation of these hard plaques.
- Course: It consists of two phases. The active phase is usually painful and progressive. The stable phase is when the pain subsides but the curvature becomes permanent.
Symptoms and Clinical Course
The severity of symptoms depends on whether the curvature is congenital or due to Peyronie's disease.
Painful Erection and Shape Changes:
- Plaque Formation: Hard, patch-like tissues (plaques) that can be felt under the skin form on the penile shaft.
- Deformities: Deformities such as an "hourglass" appearance (narrowing) or shortening of the penile length may develop.
Relationship with Erectile Dysfunction: As the curvature progresses, plaques can prevent blood from being trapped in the penis, leading to a decrease in erection quality (erectile dysfunction).
Diagnostic Methods
The diagnostic process at Adana Special Life Center includes the following stages:
- Physical Examination: The location and size of the hard plaques are determined.
- Erection Test / Photodiagnosis: Measurements taken to clearly assess the degree of curvature provide "gold standard" data for surgical planning.
- Penile Doppler Ultrasonography: Used to evaluate the degree of calcification in plaques and blood flow to the penis.
Treatment Approaches and Surgical Protocols
Treatment is tailored based on the stage of the disease:
- Medications and Injections (Active Phase): During the progressive stage, oral treatments or intralesional injections aimed at reducing inflammation are preferred.
- Surgical Techniques (Stable Phase): Nesbit/Plication methods (equalizing by shortening) or Grafting techniques (preserving length by adding tissue) are performed.
Post-Operative Recovery and Care
- First 48 Hours: Edema control and pain management are provided.
- Recovery Period: Heavy physical activities should be avoided for the first 10 days, and maximum attention should be paid to hygiene.
- Sexual Rest: To allow the tissues to heal, sexual intercourse and masturbation are strictly prohibited for 6 weeks.
Risk Management and Complications
Potential risks, though rare, include: slight shortening of the penile length (in plication techniques), temporary sensory changes (numbness), and the risk of recurrence.
